Clinical Population Monitoring provides a mechanism to simultaneously assemble risk-adjusted outcomes along with both evidence-based care process measures and segmentation measures for important clinical populations in a manner that enables evaluation of possible root-causes and potential solutions with minimal additional study. Clinical Population Monitoring (CPM) will incorporate condition-specific reporting with new aggregate diagnosis, procedure, and transaction measures.
