Clinical Population Monitoring, Linking & Filter 34 min Web Seminar (Recorded)

Description: In this session, participants are given a overview of comparative reporting for population specific measures in Population Finder and Reveal. Clinical Population Monitoring provides a mechanism to simultaneously assemble risk-adjusted outcomes along with both evidence-based care process measures and segmentation measures for important clinical populations in a manner that enables evaluation of possible root-causes and potential solutions with minimal additional study. Clinical Population Monitoring (CPM) will incorporate condition-specific reporting with new aggregate diagnosis, procedure, and transaction measures.
